Last month, I assisted a family in securing and transferring a patient into a long-term care facility. I have been working with this family for over a year. The process was challenging due to the patient’s behaviors and past history at other facilities, including aggression and outbursts. Additional barriers included the patient’s age (40s) and previous facilities declining placement due to their HD diagnosis. I was able to help secure placement at a local facility, and the family is planning to engage in our in-service training on 5/22 to educate staff on HD and how to provide the best possible care for the patient.
