 2026 North Carolina Education Day Application for Travel / Gas Stipend

This form must be completed in full and submitted to HD Reach as your “application” for the 2026 North Carolina Huntington’s Disease Education Day. All information contained on this application will be kept in strict confidence. Support is available until funds are depleted.

Deadline: Monday, March 30, 2026

Determination of Travel / Gas Stipend Awards: You will be notified no later than April 6, 2026 as to the status of your application.

***You must be age 18 or over to receive a travel voucher. ***

***Only 1 per family in the same household.***

***Exhibitors and clinicians are not eligible for a travel stipend.***

Hotel stipends will be paid after HD Reach receives a copy of your hotel receipt.

Gas stipends will be paid after your attendance at the event is confirmed.

*Special circumstances may warrant receiving both a hotel and a gas stipend.
